The European Union (EU) has brought unprecedented benefits to its citizens by facilitating the freedom of movement for persons, services and goods. Despite these achievements, not every European citizen personally experiences the advantages of the EU equally. European citizens living in border regions (around30% of EU population) still perceive a distance between the European idea and its realization in their daily lives. Border regions are less economically and socially developed, perhaps because European integrationhas mainly been a collaboration of the capitals. Thus, the EU would profit immensely from practical policies to make the benefits of the Union more visible and tangible for their cross-border citizens. Health is a tangible topic recognized as a central issue in life among everybody through Europe. However, contrary to goods, capital, services and labour, that can move freely, the borders are still visible in healthcare issues.  

The recent corona crisis showed how real the borders are and how different the policy on each side of theborder can be. At the same time, the strength of European solidarity has been evident, e.g. transferring patients to intensive care units across the border when capacities are exhausted. Certainly, containing the spread of the corona virus and taking care of severely ill covid-19 patients in border regions, demand coordinated actions and resolute cross-border agreements. This form of cross-border cooperation can ensure more efficient use of public and private resources and can create prosperity and well- being for theinhabitants.  

Nevertheless, cross-border healthcare activities and structures are still unusual in most border regions like the Northern Dutch-German border region. In this particular border region, the population’s socio-economic status is rather low, which has been shown to lead to a high prevalence of overweight and obesity among adults. Therefore, the Aletta Jacobs School of Public Health in Groningen, NL, together with the School of Medicine and Health Sciences at the University of Oldenburg, DE, joined efforts to create the Cross-border Institute of Healthcare Systems and Prevention (CBI), which started its activities on February 2019.

From the CBI we are committed to tackle the different cross-border health challenges that our region faces to offer border citizens dignified healthcare conditions. It is crucial for us to find evidence-based solutions and to identify the best preventive-driven health incentives. This can be done by comparing our Dutch andGerman health(care) systems, drawing conclusion for major future health challenges and using it for translation and implementation. To achieve this goal, we need the participation and commitment of different stakeholders.  

Thus, we are inviting health insurance companies in the region, health policy makers, health authorities inthe province of Groningen, Drenthe, Friesland and in the Weser-Ems region in Germany to join and support the CBI effort. Cross-border cooperation in health is an opportunity to jointly overcome the healthcare challenges in the northern region in a comprehensive way. It also represents an important prospect for health insurance companies on both sides of the border to collaborate more intensively with each other and with each other’s region – potentially even leading to financial benefits.  

For more than 40 years the Ems Dollart Region has been committed to cooperation between residents, companies, authorities and organisations throughout the region. In addition to providing a scientific basis for presenting and researching joint solutions to the challenges facing the Ems Dollart Region, it is essential forresidents that high-quality care remains accessible in the region. On the other hand, the cooperation between care institutions and nursing staff in both countries contributes to ensuring that the often complex and costly care remains efficiently available to residents throughout the region. This contributes to the aim of the Ems Dollart Region and its members to promote cooperation throughout the region so that residents can take full advantage of the opportunities offered by both countries. By ensuring that residents have access to high quality and affordable care throughout its 360-degree catchment area, the viability of the region is assured and specialist care remains accessible throughout the region.

Cross-border cooperation in healthcare has never been so important and needed as now. It is the time to give the border regions an active push, with the aim to make them more resilient to crisis situations, but also toprepare them better for the more gradual changes that are coming due to globalization, digitization and the demographic transition.

Position paper namens het grensoverschrijdende Cross-border Institute of Healthcare Systems and Prevention (CBI) & Eems Dollard Regio (EDR)

 
De Europese Unie (EU) heeft haar burgers grote voordelen gebracht door het vergemakkelijken van het vrije verkeer van personen, diensten en goederen. Ondanks deze verworvenheden ervaart niet elke Europese burger de voordelen van de EU in gelijke mate. Europese burgers die in grensregio's wonen (ongeveer 30% van de EU-bevolking) zien nog steeds een afstand tussen de Europese gedachte en de verwezenlijking ervan in hun dagelijks leven. Aan de ene kant is het oversteken van de grens in deze regio’s natuurlijker, aan de andere kant zijn grensregio’s vaak economisch en sociaal minder ontwikkeld. Grensoverschrijdende samenwerking die een direct voordeel heeft voor de burger, zou een nieuwe impuls geven aan het idee van Europese eenwording. Vooral de samenwerking op het gebied van gezondheidszorg is veelbelovend.

De recente coronacrisis heeft aangetoond hoe echt de grenzen zijn en hoe verschillend het beleid aan elkekant van de grens kan zijn. Tegelijkertijd is de kracht van de Europese solidariteit duidelijk geworden, bijvoorbeeld door het overbrengen van patiënten naar intensive care-afdelingen over de grens bij te weinig eigen capaciteit. Het indammen van de verspreiding van het coronavirus en het verzorgen van ernstig zieke coronapatiënten in grensregio's vragen om gecoördineerde acties en resolute grensoverschrijdende afspraken. Deze vorm van grensoverschrijdende samenwerking kan zorgen voor een efficiënter gebruik van openbare en particuliere middelen en kan welvaart en welzijn voor de bewoners creëren.  

Al meer dan 40 jaar zet de Eems Dollard Regio zich in voor samenwerking tussen burgers, bedrijven, overheden en organisaties in de Noord-Nederlands-Duitse grensregio. Ondanks vele inspanningen zijn grensoverschrijdende gezondheidszorgactiviteiten en -structuren nog steeds ongebruikelijk in de meeste grensregio's, waaronder de Noord-Nederlands-Duitse grensregio. De bevolking wordt gekenmerkt door een vrij lage sociaaleconomische status die gepaard gaat met tal van gezondheidsproblemen.

Daarom hebben de Aletta Jacobs School of Public Health in Groningen (NL) samen met de School of Medicine and Health Sciences van de Universiteit van Oldenburg (DE) de krachten gebundeld om de Cross-border Institute of Healthcare Systems and Prevention (CBI) op te richten, dat in februari 2019 gestart is met haar activiteiten. Vanuit de CBI en de EDR zetten wij ons in om de verschillende grensoverschrijdende gezondheidsuitdagingen waar onze regio mee te maken heeft aan te pakken en de grensbewoners een waardige gezondheidszorg te bieden. Het is voor ons van cruciaal belang om evidence-based oplossingen te vinden en de beste preventieve gezondheidsstimulansen te identificeren. Dit kunnen we doen door onze Nederlandse en Duitse gezondheids(zorg)systemen te vergelijken. Bovendien zullen we analyseren welke strategieën succesvol zijn en zullen we evidence-based concepten ontwikkelen voor grote toekomstige uitdagingen in de gezondheidszorg. Om dit doel te bereiken, hebben we de deelname en betrokkenheid van verschillende belanghebbenden nodig. Daarom nodigen we zorgverzekeraars in de regio, zorgbeleidsmakers, gezondheids- autoriteiten in de provincie Groningen, Drenthe, Friesland en in de Weser-Ems regio in Duitsland uit om zich aan te sluiten bij de CBI-inspanning en deze te ondersteunen.

Voor burgers in de grensregio is hoogwaardige gezondheidszorg essentieel. Om dit in de toekomst zeker te stellen is samenwerking tussen zorgorganisaties en zorgaanbieders in beide landen nodig. Met deze aanpak draagt de CBI bij aan het doel van de Eems Dollard Regio en haar leden om samenwerking in de hele regio te bevorderen, zodat burgers ten volle kunnen profiteren van de kansen die beide landen bieden. Door ervoor te zorgen dat burgers in grensregio's gebruik kunnen maken van hoogwaardige en betaalbare gezondheidszorg - ook aan de andere kant van de grens - wordt de leefbaarheid van de regio, de toegang tot gezondheidszorg en daarmee een hoge kwaliteit van leven voor de burgers gegarandeerd. Grensoverschrijdende samenwerking is een uitdaging, vooral voor zorgverzekeraars aan weerszijden van de grens, maar zal op de lange termijn waarschijnlijk leiden tot financiële voordelen.

Grensoverschrijdende samenwerking in de gezondheidszorg is nog nooit zo belangrijk en noodzakelijk geweest als nu! Ons doel is om de regio beter bestand te maken tegen crisissituaties en hen voor te bereiden op de veranderingen die gepaard gaan met globalisering, digitalisering en de demografische verandering. De EDR en de CBI willen actief een aanzet geven om dit doel te bereiken. We rekenen op steun uit de regio!
