Summary:

Main goal of the Diabetic project Friesland North is an all-embracing care in primary health care. Quality of life is expected to improve, also delay of complications development is expected. This care maintains by others a clear cooperation and task delegation between general practitioner, nurse practitioner, diabetic nurse and dietician. Also a registration method, Diabcare, is used. With Diabcare benchmarking is possible. The patient will achieve a diabetic passport for patient participation stimulation.

The central question is: What is the effectiveness and efficiency of the Diabetic project Friesland North? Health profit, process aspects and cost aspects will be studied.
